As of 2018, the world has more senior citizens than children under 5 for the first time in recorded history.  We are facing a global demographic shift of epic proportions, one that the history books provide no guidance for.  There are approximately 52 million seniors out of 327 million people in the USA today, that’s 16% of our national population. Seniors 65 and older will double in the next 15 years and those over 80 triple and those over 90 will quadruple by mid-century.  They call this wave of elderly the “Grey Tsunami.” In Japan and Western Europe this wave has already hit.  Japan has 27% of its population as senior citizens today and is projected to grow to more than 40% seniors by mid-century.   Japans total population has shrunk by 2 million people in the last 5 years due to younger adults not reproducing.  Less people are in the workforce per capita and a record number of social services are required to support retirees.  Right behind Japan and catching up quickly is China with its 1.4 billion people.  China will have more seniors than the entire U.S. population, an alarming prospect.  
  
The family unit is no longer capable of managing the health and wellness of our beloved seniors.  Today’s seniors, particularly as they become frail or approach 80, require highly specialized care yet need social structure to remain relevant and part of society.  This requires an integrated approach, something very different than what you see today.   As a means of background, the USA spent $4 trillion on healthcare for its 320+ million people in 2017, that’s more than the entire GDP of Germany as a Nation for the same period.   The USA is ranked 31 (low) among nations in terms of quality of care in spite of spending more than any nation on earth. This will be the largest issue facing all developed nations.  Economies of the developed world, including leading nations, will be brought to their knees if the current system is perpetuated without significant change taking place.  Sustainable solutions are needed.

As a Platform, AilaOS is designed to pair with IoT devices, particularly medical and wellness smart sensors which come in the form of electronic band aids wirelessly affixed to seniors.  Floor tiles, weight scales, sinks, toilets and beds are sensitized in such a way that key vital signs are ingested by AilaOS Platform routinely day and night unobtrusively.  iDeskMD ends up with unprecedented real time data 24/7 that’s fed into Machine Learning Algorithms to produce actionable data.   Artificial Intelligence learns what combinations of events indicate a condition is emerging that would otherwise cause the elderly person to become hospitalized or worse.  AilaOS alerts the entire care team of the elder’s condition and trajectory in advance of the otherwise imminent hospitalization, thus creating interventions routinely, improving quality of life, saving lives and reducing cost and burden to the medical care system.  This system can aid clinicians in some cases up to a month ahead of hospitalization.  This proprietary technology developed by iDeskMD will be invaluable to Medicare, Medicaid and to Health Insurance payers. 

Seniors decline from simple conditions such as dehydration and can be revived without hospitalization, the most expensive and debilitating process avoided.   Healthcare today for seniors is highly reactive, with so many seniors sent directly to the Emergency Room for health care. After the hospital, the system frequently sends them to third party Skilled Nursing Facilities, all at a tremendous cost to tax payers and the system, not to mention the grief of the family and the elderly themselves.  The pearl of wisdom that I learned from speaking to many 80 and 90+ year olds is that no one wants to die in the hospital, and key is to “live in place” and support the processes that empower this.  Its hospitalization and “transitions of care” that are so painful and potentially deadly to the frail elderly population when avoidable.  It’s not the same as driving a middle-aged person to the Doctors. Moving frail elderly to different clinical settings is hazardous. From a cost perspective, it’s not uncommon for a senior to have a half-million dollars of medical expenses for what looked like a basic problem, given the lack of interoperability in today’s Electronic Health Records.
