As for the old man (Skousen), he makes a couple of valid points, but overlooks the most obvious reason why it will not happen:
If ONE nuke flies and destroys a city,… they all fly.
This means their attempt for a “limited” nuclear exchange goes all pair shaped and most of the planet is left uninhabitable for say the next 200~300 years or so.
This simple realty does not reflect the fact that their are some real psycho’s embedded in the command chain that feel it is better to destroy ALL of humanity, then to not get their way. Sick,.. yes I know, but that is the wild card in this very possibility of materializing this kind of event.
