French Academie’ of Sciences (Link Below) – Here’s the TRANSLATION: “Covid-19: the hypothesis of the central role of the acetylcholine nicotine receptor and its preventive and therapeutic implications — Researchers from the CNRS, Inserm, Public Assistance – Hospitals of Paris, Sorbonne University, Collège de France and Institut Pasteur publish an article in the Journals of Biology of the Academy of Sciences that postulates a central role of the nicotinic receptor of acetylcholine in the propagation and pathophysiology of Covid-19.
The researchers’ hypothesis is based on the combination of two different but complementary scientific approaches.
A first observation, made as soon as the initial series of Covid-19 patients were published, is that of a low rate of smokers in this population. This signal, which was impossible to interpret due to numerous methodological biases, prompted a French study taking into account these confounding factors which confirmed that active smokers are protected against SARS-Cov-2 infection (Miyara M et al. Submitted for publication; preprint available on Qeios). The reasons for this protection are not established but nicotine could be a candidate.
A second follow-up finding suggests that SARS-CoV-2 infection involves the nicotinic receptor of acetylcholine:
- the high prevalence of neuropsychiatric manifestations during Covid-19 is in favour of SARS-CoV-2 neurotropism. SARS-CoV-2 could spread from the olfactory mucosa and then from the neurons of the brainstem, in some cases to respiratory centres. This invasion would be accompanied by the loss of the sense of olfaction, and in some patients, various neurological disorders until, possibly, a sudden respiratory arrest occurring in a staggered manner (explaining the turn observed around the 8th day). In addition, the SARS-CoV-2 envelope exposes a loop with a sequence similar to a pattern found on the glycoprotein of the rabies virus. This one is known for a neurotropism directly related to its fixation on the nicotinic receptor of the nerve-muscle junction. It enters motor neurons and then spreads to the central nervous system where it creates serious behavioral disorders. These sequence elements are also similar to a pattern found on a snake venom toxin, bungarotoxin, whose strong affinity for the receptor – the hyper-inflammatory state and cytokinic storm described in severe Covid-19 patients could also be explained by the intervention of the nicotinic receptor. Acetylcholine has a regulating effect of inflammation by its action on the macrophagic nicotine receptor. The disorder of this receptor causes macrophagic hyperactivation with secretion of proinflammatory cytokines as observed in Covid-19 patients. This alteration of the nicotine receptor is the cause of the inflammatory residual condition described during obesity and diabetes, which could be amplified in the event of SARS-CoV2 infection. This hypothesis would explain why these two comorbidities are so frequently found in severe cases of Covid-19.
Given the health emergency, it seems desirable to quickly assess the therapeutic impact of nicotinic receptor modulators, direct and/or indirect, addictive or nonaddictive on SARS-CoV-2 infection. Clinical studies are underway.”
