Tooth implant warning linked to a bone-destroying infection in 1 in 5 patients
(NaturalHealth365) Many dentists present implants as the permanent answer to a missing tooth. Once the post fuses with the jawbone, many patients assume the job is done for life.
Yet a major review of more than 13,000 implant patients tells a more complicated story. In fact, about one in five went on to develop an infection that eats away at the bone holding the implant in place.
A common problem most patients never hear about
For the review, published in 2025 in the Journal of Periodontology, researchers from Harvard, Tufts, and the University of Pennsylvania pooled 102 clinical studies. Together, those studies followed 13,030 people with dental implants.
About 46% of patients developed peri-implant mucositis, a gum inflammation around the implant that can still be reversed. Meanwhile, roughly 21% developed peri-implantitis, a deeper infection that destroys the supporting bone. Over 20 years with an implant, the share with peri-implantitis climbed to about 22%.
The risk factors you can actually change
A history of gum disease was the most consistent risk factor for both conditions. Smoking also raised the risk of each, while diabetes was tied to the bone-destroying form of the infection. Additionally, obesity went along with more gum inflammation, and alcohol with more peri-implantitis.
The encouraging news is that most of these risks respond well to everyday choices. Healthy gums before surgery, steady blood sugar, and a smoke-free mouth all give an implant a better foundation. As a result, preparing your body matters as much as choosing a skilled dentist.
Metal particles turned up around every implant
Many implants are made of titanium, a strong metal that is in contact with bone. In a 2025 study published in Communications Medicine, Swedish researchers examined gum tissue taken from around the implants of 21 patients. Strikingly, they found tiny titanium particles at every implant they examined, including healthy ones.
Tissue with more titanium particles showed changes in 14 genes tied to inflammation and wound healing. However, particle levels did not differ between healthy and infected sites in the same person. So the study raises important questions about metal debris without proving that the particles cause infection.
Why biological dentists often choose zirconia
Many biological dentists prefer zirconia implants, a white, metal-free ceramic. Because zirconia is a ceramic, the material does not corrode and shed metal particles the way titanium can. Zirconia also blends with the natural color of teeth and gums.
Two randomized trials published in 2026 suggest zirconia holds up well. In a multicenter trial of 122 implants, every zirconia implant survived the first year, compared with 96.5% of titanium implants, and bone loss was similar for both.
Meanwhile, a three-year trial of 50 patients found nearly identical bone levels, and patients with zirconia gave higher ratings for the color of their gums.
Simple ways to protect your gums and implants
Heal gum disease before you get an implant. Since a history of gum disease was the strongest risk factor, treat any bleeding or receding gums first. Ask your dentist to check your gum health and bring inflammation under control before surgery. A healthy starting point gives the implant a much better chance of lasting.
Ask about metal-free zirconia. If you need a tooth replaced, ask whether a zirconia implant fits your situation. A biological or holistic dentist can walk you through the options for your mouth and bite. Knowing your choices before the procedure puts you in control.
Keep blood sugar steady and skip tobacco. Diabetes and smoking both raised infection risk in the review. Cutting back on added sugar and refined carbs, moving after meals, and avoiding all forms of tobacco help protect gum tissue. Limiting alcohol also lowers the odds of the bone-destroying form of infection.
Book cleanings and nourish your gums. Professional cleanings every three to six months help catch problems early. At home, an oral water irrigator or small interdental brushes can clear plaque around the implant base. Vitamin C helps build healthy gum tissue, while vitamin D supports a strong jawbone. Organic citrus, bell peppers, leafy greens, wild-caught salmon, and quality supplements all help.
What to know before your next dental appointment
An implant can restore a smile and make chewing comfortable again. Yet the research shows that what happens after surgery, and what material goes into your jaw, matters for years to come. So before you sign up for an implant, have you asked what the post is made of?
The materials in your mouth affect far more than your smile. Jonathan Landsman’s Dental Health Docu-Class explores the best approaches to dental care, including safer choices for restoring teeth, reversing gum disease and detecting hidden infections.
