Artery plaque showed up in 1 in 13 adults under 30 who felt healthy

Neck ultrasound checking a young adult for silent artery plaque(NaturalHealth365) Most people picture clogged arteries as a problem for retirees. Yet a major new study found artery plaque in about 1 in 13 adults between ages 18 and 29, all of whom had no known heart disease.

Those young adults felt perfectly healthy. Even more troubling, the standard tools doctors use to estimate heart risk failed to flag most of the people who already had plaque.

Undetected artery plaque inside more than half of “healthy” adults

For the study, published in August 2026 in the New England Journal of Medicine, researchers in Denmark and Spain examined 16,808 adults ages 18 to 70. None had a history of heart attack, stroke, or other artery disease. Each person received detailed imaging of the neck arteries, the leg arteries, and the arteries that feed the heart.

Overall, 57.1% of participants already had plaque in at least one of those areas. In the youngest group, 8.7% of men and 6.7% of women showed plaque. By ages 60 to 70, about 9 in 10 people carried plaque somewhere in their arteries.

Plaque often shows up outside the heart first

In younger adults, plaque usually appeared in just one area, most often the neck or leg arteries. With age, the amount of plaque grew sharply, and more people had buildup in several areas at once.

That pattern matters for early detection. Most people with plaque in the heart’s arteries also had plaque in the neck or legs, and an ultrasound of those vessels is quick, painless, and free of radiation.

Women face a steep rise during midlife

Men tended to develop plaque five to 10 years earlier than women. However, women caught up quickly, with the sharpest increase between ages 40 and 60.

That window lines up with the menopausal transition. So women who felt protected in their 30s have good reason to pay closer attention to artery health during midlife.

Standard risk scores missed most people with plaque

Doctors usually estimate heart risk with a formula based on age, blood pressure, cholesterol, and smoking. In this study, those risk scores identified only a small share of the people who already had silent artery plaque.

In other words, a reassuring risk score did not guarantee clean arteries. A recent NaturalHealth365 report described a similar blind spot, where people with fatty liver carried more unstable soft plaque even after researchers accounted for standard heart risk scores.

Small daily habits matter more than most people think

Because plaque can begin decades before symptoms, daily choices carry real weight long before middle age. Encouragingly, research on small changes found that a little more sleep, a few extra minutes of brisk movement, and a bit more produce were linked to fewer heart attacks and strokes.

Earlier research led by the same Spanish heart research center also tied plaque in middle-aged adults to lifestyle patterns. So the habits you build in your 20s, 30s, and 40s can shape what your arteries look like decades later.

What the findings do and do not show

This study captured a single snapshot of plaque, not what happened to participants afterward. As a result, the findings cannot predict a heart attack or stroke with 100% certainty.

More importantly, we should view the study results as a hopeful message.  In younger adults, plaque tended to be small and limited to a single artery, while the total amount of plaque climbed steeply in later decades. That pattern points to early adulthood and midlife as the most promising window to slow buildup, before small deposits become widespread disease.

Practical ways to protect your arteries starting now

Ask about a simple artery ultrasound. If heart disease runs in your family, or you want a clearer picture despite normal lab numbers, ask your doctor whether a carotid or femoral ultrasound makes sense. The test is painless and uses no radiation.

Skip tobacco in every form. Smoking remains one of the strongest drivers of artery damage. Quitting cigarettes helps protect artery walls at any age, and vaping is no safe substitute.

Move your body every day. Brisk walking, stair climbing, cycling, and strength training all support healthy blood vessels. Several short sessions add up just as well as one long workout.

Fill your plate with whole organic foods. Leafy greens, colorful vegetables, berries, nuts, olive oil, and wild-caught fish make a strong foundation. Meanwhile, cutting back on sugary drinks and heavily processed foods eases the burden on your blood vessels.

Women, give midlife extra attention. The years around menopause brought the steepest rise in plaque for women in this study. That stage makes a smart time to review heart health with a trusted practitioner.

Healthy arteries start long before middle age

Simply put, artery plaque is far more common, and starts far earlier, than most people realize. The encouraging part is that every year of healthy habits gives your arteries a better chance to stay clear. So what small step could you take today for the arteries you will rely on 30 years from now?
